SERVICES
I am Nanette Skiba, sole proprietor of Nutrient-Knowledge.com, and am registered with the City of Boston Small Business Unit. I offer online nutritional consultations and also conduct in-person educational seminars for groups. I use HIPPA-compliant software and conduct my online appointments in privacy. I do not record my sessions and expect my clients to honor this practice. Clients receive a summary of the visit within 48 hours, and my notes are secured in a Cloud-based storage file that is based on a numerical system, not client names.
PERSONAL RESPONSIBILITY
Clients requesting both short or full-term appointments must complete informational forms, prior to appointment, for full review. A list of any and all medications and or supplements should be included to avoid any contraindications regarding diet, medications, or supplements. Please state clearly your reason for seeking the services of a nutritionist. If you have a recent diagnosis from your physician, this is required as well. Clients are required to use the same email address during the entire process, and they are responsible for forwarding consultation information to their providers. I do not consult with physicians’ offices regarding client appointments.
ASSUMPTION OF RISK
So many factors affect our health that the word “cure” is inappropriate in the context of this work. My goal is to find solutions so that clients can function at an optimal level in a complex world.
PAYMENT
Services must be paid in full at least 72 hours prior to appointment, using Stripe, an online payment platform. I do not bill clients or insurance companies. Single-question appointments are booked for 30 minutes maximum. Full-length appointments are booked for a minimum of one hour but may go longer at no extra charge. Appointments canceled less than 48 hours before the scheduled time will be charged half price. If a personal emergency requires me to cancel a client’s appointment, the session will be rescheduled to accommodate the client’s needs if possible, and if the client is not satisfied, then a full refund will be available.
